with moderate to severe canal stenosis at this level. 2. Annular bulging at L4-L5 and L5-S1 with mild narrowing of the central canal at L4-L5. 3. Mild to moderate neural foraminal stenosis at L5-S1 with left greater than right.
Diagnosis: Lumbar disc bulge L3-L4 with moderate to severe central canal stenosis

Diagnosis: Right paracentral posterior lateral disc herniation at L4-L5 with resultant lumbar radiculopathy
